Who is Eligible For Hospice Care?
Any patient with a limited life expectancy prognosis As determined by the patient's Physician and Hospice Medical Director As long as the patient's prognosis is within Medicare or other insurance guidelines, they may continue to receive hospice services

Routine Home Care
Provided to help with managing pain symptoms, to make patient comfortable. Care may be provided in a facility or in a home.
Continuous Care
Provided to patients that are in need of continuous attention due to periods of acute pain.
Hospice In-Patient Care
Provided to help with managing pain symptoms, to make patient comfortable. Care may be provided in a facility or in a home.
